Job summary

Qualifications

• Bachelor's degree in a generally recognized clinical discipline.
• Case Manager I requires 2 years' experience in case management or drug/alcohol counseling.
• Case Manager II requires 3 years' experience in case management or drug/alcohol counseling.
• Case Manager III requires 3 years' experience including at least 1 year supervisory/administrative experience.

Responsibilities

• Treat all clients and staff with dignity and respect.
• Attend all team/staff meetings as required by Supervisor.
• Role model pro-social behavior.
• May conduct intake interviews as needed.
• Maintain a caseload as identified by program requirements and state regulations.
• Complete progress notes for all client-related activities within 24 hours of service delivery.
• Coordinate with agency and community resources to ensure thorough treatment and discharge plans.
• Establish and maintain positive relationships with service providers and referral sources.
• Assist clients in connecting to resources to reach personal goals during treatment and before discharge.
• Arrange access to public assistance, social security, food stamps, and medical assistance.
