Job summary

Qualifications

• Associate's Degree in a Human Service field or CAC certification required.
• Bachelor's degree preferred; Master’s not required.
• CAC certification with experience strongly preferred in addiction treatment.

Responsibilities

• Provide case management, discharge planning, and care coordination for ASAM-based programs.
• Handle intake services, prepare reports, treatment and discharge/continuing care plans, and maintain records.
• Coordinate activities for client groups; generate referrals and aftercare plans.
