Job summary

Penn State Health is seeking a Hospital at Home RN in Hershey, PA. This part-time, 0.50 FTE daytime role coordinates and evaluates complex at-home care delivery with advanced clinical expertise, education, and research contribution.

You will collaborate with telemedicine physicians, pharmacists, respiratory therapists, and care teams, leading virtual rounds and guiding staff to ensure high-quality, evidence-based patient outcomes.

Qualifications

• Associate Degree in Nursing.
• Current licensed RN in state of employment or hold multistate RN license via NLC.
• Two (2) years nursing experience in Case Management, Care Coordination or relevant specialty.
• AHA BLS required prior to end of orientation period.

SUMMARY OF POSITION:

The hospital at home RN coordinates and evaluates complex care delivery projects, providing advanced clinical expertise, education and contributing to research within their specialist. This role facilities change and collaborates closely with tele Hospital Physicians, APPS, Pharmacists, respiratory therapists and patient placement teams to manage all aspects of hospital-level care at home admissions and transfers. The RN conducts virtual rounds to ensure adherence to evidence-based practices and proactively provides clinical leadership, mentorship and support for resource pool and ancillary program nurses, driving high- qualify outcomes in a dynamic care environment.

MINIMUM QUALIFICATION(S):

• Associate Degree in Nursing
• Currently licensed to practice as a registered nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact
• Two (2) years nursing experience in Case Management, Care Coordinator or relevant specialty area
• AHA BLS prior to end of orientation period
