Qualifications

• Bachelor’s degree in psychology, human services or related field required.
• One year of relevant experience preferred with sensitivity to drug-dependent populations.
• Must possess a valid driver’s license and a safe/clear driving record.

Responsibilities

• Acts as the primary contact for residents accessing services at the treatment facility.
• Conducts comprehensive assessments and develops individualized service plans.
• Coordinates referrals to internal and external resources, including treatment programs and community agencies.
• Advocates for patients to ensure access to needed services and resources.
• Monitors patient progress and updates service plans as needed.
• Maintains up-to-date patient records and documentation.
• Collaborates with clinical and medical staff for coordinated care.
• Leads or co-facilitates support groups for patients in recovery.
