Root Center for Advanced Recovery is seeking a Care Coordinator to promote engagement in treatment and facilitate discharge planning to connect residents to the next level of care. The role emphasizes coordination with clinical staff, assessment, and advocacy to support recovery.

Competenze

• Bachelor’s Degree in psychology, human services or a related field required. One year of relevant experience preferred. Must possess a valid driver’s license and safe/clear driving record.

Mansioni

• Acts as the primary point of contact for residents accessing services.
• Conducts comprehensive assessments of patients’ needs and develops individualized service plans.
• Coordinates referrals to internal and external resources.
• Advocates for patients to ensure access to needed services and resources.
• Monitors patient progress and adjusts service plans as needed.
• Maintains up to date patient records and documentation.
• Collaborates with clinical and medical staff to ensure coordinated care.
• Leads or co-facilitates support groups for patients in recovery.
• Ensures patients’ insurance is active and up to date; assists with insurance lapse notifications and applications.
• Collects fees and records payments; supports billing and insurance authorization; prepares medical records for admissions; updates lobby boards; handles late fee letters.
