Job summary

Qualifications

• Bachelor’s Degree in psychology, human services or a related field required. One year of relevant experience preferred. Must possess a valid driver’s license and safe/clear driving record.

Responsibilities

• Acts as the primary point of contact for residents accessing services.
• Conducts comprehensive assessments of patients’ needs and develops individualized service plans.
• Coordinates referrals to internal and external resources.
• Advocates for patients to ensure access to needed services and resources.
• Monitors patient progress and adjusts service plans as needed.
• Maintains up to date patient records and documentation.
• Collaborates with clinical and medical staff to ensure coordinated care.
• Leads or co-facilitates support groups for patients in recovery.
• Ensures patients’ insurance is active and up to date; assists with insurance lapse notifications and applications.
• Collects fees and records payments; supports billing and insurance authorization; prepares medical records for admissions; updates lobby boards; handles late fee letters.

Position Summary

The Care Coordinator promotes engagement and retention in treatment and facilitates an effective discharge plan whereby the individual successfully connects to the next level of care.

Hourly Rate Range

Hourly Rate Range: $22.00 to $30.00

Second Shift and Weekends as needed.

Essential Functions

• Acts as the primary point of contact for residents accessing services at the treatment facility.
• Conducts comprehensive assessments of patients’ needs and developing individualized service plans.
• Coordinates referrals to internal and external resources, including treatment programs, support services, and community agencies.
• Advocates for patients to ensure access to needed services and resources.
• Monitors patient progress and adjusts service plans as needed.
• Maintains up to date patient records and documentation.
• Collaborates with clinical and medical staff and other service providers to ensure coordinated care.
• Leads or co-facilitates support groups for patients in recovery, providing a safe space for sharing experiences, discussing challenges, and offering mutual support.
• Ensures patients’ insurance is active and up to date. Meets with patients if insurance is going to lapse and completes applications as needed.
• Collects fees and records payments; coordinates accounts receivable to include the billing and insurance authorization processes; electronically prepares medical records for new admissions and accompanying information; processes client refunds; participating in clinical staff meetings; DDAP processing for DMHAS client tracking requirements admissions and discharges; completes client fee contracts; updates lobby boards; process late fee letters on a timely basis

Qualifications/Skills Required

A Bachelor’s Degree in psychology, human services or a related field required. One (1) year of relevant experience preferred with sensitivity to work with drug dependent people within the scope of this rehabilitation program. Must possess a valid driver’s license and safe/clear driving record.
