Job summary

Granite Recovery Center is seeking a Recovery Case Manager to work full-time in New Hampshire. You will meet clients one-on-one, coordinate with families and referrals, and implement aftercare plans.

The role emphasizes documentation, compliance, and clear communication within a dynamic treatment environment. Expect collaboration across departments, weekly client meetings, and scheduled contacts with families.

Qualifications

• High school diploma or GED required.
• Extensive experience in mental health and substance use disorders.
• Strong time management and multitasking skills.
• Coordinate with departments and oversee client stay.
• Create after-care plans meeting client needs.
• Complete background checks and screenings as required.

Responsibilities

• Meet clients one-on-one regularly.
• Primary contact for families and referral sources.
• Plan and implement aftercare.
• Log calls and meetings in EMR.
• Schedule family and referral calls.
• Attend required meetings (Morning, RCM, Supervision, QA).
• Conduct weekly client meetings and progress reviews.
• Discharge planning and step-downs.
