Job summary

Granite Recovery Centers in Effingham, NH seeks a Full Time Patient Engagement Coordinator to support residents at the Residential Treatment Facility. You will engage patients daily, run activity services, coordinate groups, and document interactions in the EMR to support treatment goals.

Responsibilities include crisis intervention, guiding patient participation, arranging transportation for PHP activities, and collaborating with clinical staff to enhance engagement and reduce AMA risks.

Qualifications

• High school diploma or GED.
• Ability to work in a multi-cultural, community-based organization.
• Commitment to maintain confidentiality.
• Understand the agency's mission and culture and maintain boundaries.
• Experience with substance abuse preferred.
• Employment contingent upon clear UDS and criminal history.
• Good interpersonal skills.
• Ability to lift up to 25 lbs.
• Able to ascend and descend up to three flights of stairs.
• Satisfactory drug screen and background check.

Responsibilities

• Complete EMR notes during the assigned shift in a timely manner.
• Ensure patients attend groups and do not leave without permission.
• Orient patients to the program using the patient handbook.
• Provide educational and support groups for patients.
• Assist with transport of PHP patients to off-site activities.
• Offer concierge support to detox patients and engage them in programming.
• Assist with AMA discharge rescues.
• Be available for crisis intervention.
• Document weekly the number of patients intervened with and outcomes.
• Coordinate activity services based on individual skill sets.
