Job summary

Gaudenzia, Inc. is seeking a position focused on determining level of care for individuals seeking treatment.

The role involves conducting LOC assessments, completing ASAM-based evaluations, and performing interviews in various formats. The successful candidate will document assessments in electronic health records and PA WITS, coordinate with funding sources, and follow regulatory guidelines for discharge follow-up.

Qualifications

• Two years' clinical experience with Associate's degree.
• One year clinical experience with Bachelor's degree.
• Experience conducting LOC assessments and related evaluations.

Responsibilities

• Be responsible for level of care (LOC) assessments for individuals seeking treatment, as identified by program requirements and in accordance with state regulations.
• Complete required evaluations and assessments, including screening, intake, clinical assessments, ASAM and biopsychosocial histories.
• Complete face-to-face, virtual, and/or telephonic interviews with individuals presenting for LOC assessment and/or admission to the appropriate program.
• Accurately document assessment/intake records, records of referral, and funding source approvals/authorizations, and diagnosis within the EHR system.
• Document all case management and clinical services required by the Single County Authority (SCA), into PA WITS and the EHR system.
• Maintain accurate electronic records and ensure timely completion of tasks per policy and funding source requirements.
• Maintain proficiency in assessment, intake and level of care protocols, utilizing ASAM criteria.
• Conduct admission funding reviews with payors and assist with MA applications as needed.
• Coordinate with funding sources and regulatory agencies to ensure access to services.
• Follow up with clients after discharge per regulatory guidelines and document appropriately.
