Job summary

Gaudenzia seeks a dedicated professional to perform level of care assessments for individuals seeking treatment, aligning with program requirements and state regulations.

You will conduct evaluations, ASAM-based assessments, and interviews (in person, virtual, or telephone). Thorough documentation in the EHR and PA WITS systems is required, with emphasis on timely, accurate records and funding considerations.

Qualifications

• Education: Associate's Degree with two years' clinical experience; Bachelor's degree with one-year clinical experience.
• Experience with LOC assessments and ASAM criteria and clinical interviewing experience preferred.

Responsibilities

• Be responsible for level of care (LOC) assessments for individuals seeking treatment, as identified by program requirements and in accordance with state regulations.
• Complete required evaluations and assessments, including screening, intake, clinical assessments, ASAM and biopsychosocial histories.
• Conduct face-to-face, virtual, and telephonic interviews with individuals presenting for LOC assessment and/or admission to the appropriate program.
• Accurately document assessment/intake records, referrals, funding source approvals/authorizations, and diagnoses in the EHR system.
• Document all case management and clinical services required by the SCA into PA WITS and the EHR system.
• Maintain accurate electronic records and timely completion of tasks per policy and funding requirements.
• Maintain proficiency in assessment, intake and LOC protocols using ASAM criteria.
• Conduct admission funding reviews with relevant payors and assist with MA applications.
• Participate in funding source reviews and make treatment length recommendations.
• Coordinate with agency and community resources to ensure access to up-to-date interim services.
• Maintain accurate information for follow-up in PA WITS and the EHR system.
• Establish and maintain positive relationships with funding, referral, and regulatory agencies.
• Follow up with clients after discharge per regulatory guidelines and document appropriately in PA WITS and the EHR.
