Qualifications

• LOC assessments for individuals seeking treatment, per program requirements and state regulations.
• ASAM criteria and biopsychosocial histories completed.
• Face-to-face, virtual, and/or telephonic interviews conducted for LOC assessment.

Responsibilities

• Be responsible for level of care assessments for individuals seeking treatment.
• Complete required evaluations and assessments, including screening, intake, ASAM and biopsychosocial histories.
• Conduct admission interviews and document records in EHR and PA WITS system.
• Maintain accurate electronic records and timely task completion.
• Coordinate with funding sources and ensure appropriate funding approvals.
