Job summary

Gaudenzia, Inc. is seeking an Assessors to support individuals on their recovery journey. You will conduct biopsychosocial assessments, perform risk evaluations, and determine appropriate levels of care using established criteria.

Qualifications

• Bachelor’s degree and active license or credentialing in PA (DOS or PCB) or pursuing licensure within 3 years.
• Minimum 1–3 years of experience in behavioral health, intake, or assessment services.

Responsibilities

• Conduct biopsychosocial assessments, including mental health and substance use evaluations.
• Perform risk assessments for suicide, self-harm, or harm to others.
• Determine appropriate level of care using ASAM/LOCUS/CALOCUS criteria.
• Develop initial treatment recommendations and coordinate admissions when appropriate.
• Collaborate with interdisciplinary teams including clinicians, nurses, and case managers.
• Provide crisis intervention and stabilization support as needed.
• Maintain accurate, timely, and compliant clinical documentation.
• Educate clients and families about treatment options and services.
• Facilitate referrals to community resources and continuum of care.
