Job summary

Responsibilities

• Maintain assigned caseload and provide direct care to residents, including assessments, discharge and aftercare planning, treatment updates, and weekly progress notes.
• Lead daily groups and document bio-psychosocial assessments and group notes.
• Coordinate with families and community resources for aftercare services.
• Ensure client records are complete, accurate, and timely entries.
