Job summary

High Point & Affiliated Organizations seeks an Aftercare Coordinator to manage resident caseloads, conduct assessments, and develop individualized care plans. You will facilitate groups, document progress, and coordinate aftercare services with community resources.

The role requires strong communication, teamwork, and adherence to regulations, with responsibilities spanning from discharge planning to crisis support and inter-team collaboration.

Responsibilities

  • Maintain assigned caseload; provide direct care to residents including assessments and counseling.
  • Discharge and aftercare planning; develop treatment plans and weekly progress notes.
  • Facilitate daily groups and document group notes.
  • Coordinate with external providers and community resources for aftercare.
  • Assist with crisis planning, safety checks, and MDT meetings.
  • Complete required documentation and maintain patient records in a timely manner.
  • Coordinate aftercare services and referrals; liaise with families as needed.

Job description

Program/Location: High Point Inpatient Campus - Plymouth, MA

Education: High School diploma or equivalent (GED), preferred

Pay range: $22-27/hr (depending on education and experience)

Status: Part Time

High Point & Affiliated Organizations (HPAO) is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout southeastern Massachusetts, offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. Aftercare Coordinator Requirements
  • Must present as courteous and professional at all times;
  • Demonstrated flexibility and ability to perform multiple tasks;
  • Must have excellent communication skills, including the ability to organize ideas in logical and clear fashion;
  • Knowledge of basic computer skills and software programs including the ability to learn, use and train HPAO Electronic Health Record (EHR);
  • Must have the ability to work with others in cooperative and collaborative manner;
  • An understanding of the relationship between substance use and mental health issues and/or self-help programs;
  • Understand and maintain therapeutic boundaries and reinforce rules in a respectful manner;
  • Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations;
  • TB/Flu and Covid19 Assessment;
  • Completion of the HPAO approved de-escalation program;
  • Successful completion of all trainings upon hire and annually as required.
Aftercare Coordinator Duties And Responsibilities
  • Maintain assigned caseload;
  • Providing direct care to residents – including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and completion of bio‑psychosocial assessments;
  • Making referrals when appropriate;
  • Responsible for each assigned residents’ medical records. All records are to be professional, complete and thorough with entries made in a timely manner;
  • Responsible for linking clients with aftercare services prior to discharge and aiding the resident and his/her family access services when appropriate;
  • Strengthen current and create new partnerships within the community to ensure that the clients develop the skills needed to sustain sobriety and/or housing;
  • Review patients'/family rights and responsibilities, emergency safety plans and grievance policy
  • Communicating/involving family in treatment at patient/client/resident’s request;
  • Maintain professionalism when connecting with individual’s collaterals;
  • Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Supervisor/Director;
  • Responsible for continued education in the field respective to their service level;
  • Responsible for contacting the insurance company for authorization, as needed;
  • Utilize walkie‑talkie as campus responders;
  • Respond to Codes as necessary;
  • Coordination of aftercare services, appointments and referrals.
  • Facilitation of identified groups and milieu activities as assigned.
  • Participation in MDT meetings to support patient care coordination;
  • Assistance with monitoring patient observation checks, as needed;
  • Completion of required documentation, including: consents/releases, Group Notes, Progress Notes, Contacts Logs;
  • Assistance with patient observation checks for monitoring milieu safety, as requested;
  • Supporting patients in crisis through verbal de‑escalation, 1:1 monitoring, or crisis/safety planning, as requested;
  • Attendance in supervision, staff trainings, and MDT meetings;
  • Request extensions after 90 days of treatment if needed;
  • All other duties as assigned by Program Director / Clinical Supervisor/ COO.
Aftercare Coordinator Qualifications
  • High School Diploma or GED, preferred;
  • Associate’s degree in human services, preferred;
  • One-year experience in substance use treatment and/or mental health field;
  • CPR Certification, preferred.
