Job summary

High Point Inpatient Campus in Plymouth, MA is seeking an Aftercare Coordinator to manage a caseload and provide direct care to residents, including assessments, individual counseling, discharge and aftercare planning, and treatment updates.

The role emphasizes coordinating with the treatment team, linking clients to aftercare services, documenting medical records promptly, and supporting patients in crisis with de-escalation and safety planning.

Qualifications

• High School Diploma or GED, preferred.
• Associate degree in human services preferred.
• One year experience in substance use treatment and/or mental health field.
• CPR Certification, preferred.

Responsibilities

• Maintain assigned caseload and provide direct care to residents.
• Conduct assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, and group facilitation.
• Make referrals when appropriate.
• Maintain residents’ medical records with timely, thorough entries.
• Link clients with aftercare services prior to discharge and assist family access to services.
