Job summary

4C Health in Indiana seeks a Recovery Facilitator to promote mental health recovery through a person-centered approach. You will conduct assessments, develop care plans, and coordinate case management with internal and external treatment teams to support clients with emotional/behavioral challenges.

The role includes delivering recovery coaching, engaging in treatment team meetings, and meeting weekly case-management targets. Four-day, 32-hour weeks and on-call opportunities enhance compensation.

Qualifications

• High school diploma or GED required; associate/bachelor preferred.
• Experience in behavioral health/case management is a plus.
• Willingness to perform in home/community settings and on-call roles.

Responsibilities

• Promote mental health recovery through person-centered assessment and planning.
• Coordinate case management with internal and external team members.
• Provide weekly case management and skills training to clients on caseload.
• Deliver recovery coaching in home or community settings and participate in supervision.
