4C Health is hiring a Skills Trainer in Indiana. You will assess clients, plan care, and provide direct services and skills training in a community behavioral health setting.

Role requires collaboration with treatment teams, documentation of progress, and potential mobile crisis on-call. Eligible for loan repayment, tuition assistance, and flexible benefits while supporting recovery-oriented care.

Competenze

• Bachelor’s degree in social services or mental health preferred but not required.
• High School diploma or GED accepted with relevant experience.
• Experience in behavioral health or related field is beneficial.

Mansioni

• Complete client assessments and develop care plans.
• Provide direct service, case management and skills training.
• Participate in treatment team meetings and on-call/mobile crisis as needed.
• Document progress and communicate with referral sources.
