Job summary

Agincare Group is seeking an Outreach Support Worker to assist individuals with learning disabilities, autism, and mental health needs in Bexley and surrounding areas. Qualifications

  • Must have at least 12 months of right to work in the UK.
  • Full UK Driving Licence is required.

Responsibilities

  • Encourage people to participate in community life and develop independence.
  • Support activities of daily living including social activities, personal care, routines, meal planning and budgeting.
  • Communicate effectively using individuals' preferred methods.
  • Engage and motivate people to improve their quality of life.
  • Specialise in positive behavioural support when needed.
  • Record accurately and report changes in health or circumstances to management.
  • Work as part of a team and use initiative where required.

As an Outreach Support Worker, you will provide high-quality, person-centred support to individuals with Learning Disabilities, Autism, Mental Health needs, and complex support requirements, helping them achieve their personal goals, develop life skills, and maintain meaningful connections within their community.

Where you'll be working:

Bexley, Bromley & Newham

You will provide outreach support to individuals within their own homes and local community.

What is required from you?

As part of your day to day work you will be offering the following;

  • Encouraging- people to play active roles in the communities in which they live, to encourage independence.
  • Supporting - Activities of daily Living including social activities, personal care, routines, planning and preparation of meals and budgeting.

  • Engaging - promoting and motivating people we support to enhance their quality of life
  • Specialising - supporting people who may need positive behavioural support approaches
  • Recording - keeping accurate records &reporting changes in health or circumstances of service users to management
  • Team player- Working as part of a team, as well as being able to work using your own initiative.
