Job summary

Agincare Group is seeking an Outreach Support Worker to assist individuals with learning disabilities, autism, and mental health needs in Bexley and surrounding areas. You will support people in their homes and local communities, helping them achieve personal goals, build life skills, and maintain meaningful connections.

The role focuses on person-centred support, independence, and community engagement, with a commitment to high-quality care across Agincare locations in England.

Qualifications

• Must have at least 12 months of right to work in the UK.
• Full UK Driving Licence is required.

Responsibilities

• Encourage people to participate in community life and develop independence.
• Support activities of daily living including social activities, personal care, routines, meal planning and budgeting.
• Communicate effectively using individuals' preferred methods.
• Engage and motivate people to improve their quality of life.
• Specialise in positive behavioural support when needed.
• Record accurately and report changes in health or circumstances to management.
• Work as part of a team and use initiative where required.
