Job summary

Easterseals PORT Health is hiring a Mobile Crisis Coordinator for the REACH Team serving Lynchburg, Bedford, and Appomattox, VA. You will deliver crisis intervention, stabilization, and care coordination in home and community settings, collaborating with families, providers, and partners to ensure timely, person-centered support during crises.

The role requires a Bachelor’s degree in Human Services with 1 year of relevant experience, Virginia licensure eligibility (QMHP-T preferred), proficiency

Qualifications

  • Bachelor’s degree in Human Services or related field with 1 year of relevant experience or equivalent
  • Virginia licensure eligibility with QMHP-T registration within required timeframes is preferred
  • Knowledge of behavioral health and crisis intervention services in Virginia
  • Strong assessment, communication, care coordination, and documentation skills including electronic records

Responsibilities

  • Provide crisis intervention, stabilization, and person-centered care in homes and community settings
  • Coordinate care with families, providers, hospitals, and partners
  • Maintain accurate clinical documentation, CEPPs, and data reporting
  • Participate in on-call crisis rotation, meetings, and trainings
  • Facilitate admissions, discharges, and referrals related to REACH Mobile Crisis services

Are you a license-eligible clinician who is passionate about supporting individuals experiencing behavioral health crises? At Easterseals PORT Health, we are committed to empowering individuals, strengthening communities, and providing life-changing services to those facing mental health and intellectual/developmental disability challenges. We are currently seeking a compassionate and dedicated Mobile Crisis Coordinator to join our REACH Team serving the Lynchburg, Bedford, and Appomattox, Virginia communities.

This is an opportunity to make a meaningful impact by providing crisis intervention, stabilization, and coordination of care to individuals when they need it most.

Schedule:

  • Full-Time
  • (3) 12-hour shifts (9:00 AM to 9:00 PM) and (1) 4-hour shift
  • Sunday through Wednesday

Your Role in Our Mission

As a Mobile Crisis Coordinator, you will provide behavioral health crisis intervention, stabilization, and care coordination for individuals in their homes and community settings. You’ll work collaboratively with families, providers, and community partners to ensure individuals receive timely, person-centered support during times of crisis.

In this role, you will:

  • Respond to behavioral health crises through in-person, telephonic, and telehealth interventions, helping individuals achieve stabilization and resolution whenever possible.
  • Provide crisis intervention, de-escalation, safety planning, treatment planning, and ongoing support while maintaining a person-centered approach.
  • Coordinate care with providers, families, hospitals, and community partners to ensure access to medically necessary services and continuity of care.
  • Facilitate admissions, discharges, and referrals related to REACH Mobile Crisis and Crisis Therapeutic Home services.
  • Maintain accurate clinical documentation, assessments, Crisis Education and Prevention Plans (CEPPs), service records, and required data reporting.
  • Participate in the REACH on-call crisis rotation, team meetings, trainings, and quality initiatives while upholding all agency, Medicaid, HIPAA, and DBHDS requirements.

What We’re Looking For

We’re seeking a compassionate, adaptable clinician who thrives in a fast-paced environment and is committed to making a difference in the lives of individuals experiencing behavioral health crises.

We also require:

  • A Bachelor’s degree in Human Services or a related field and at least one year of relevant experience. NOTE: Master’s degree in Counseling, Social Work, Psychology, or a related field is preferred.
  • Current Virginia licensure as an LPC, LCSW, or LMFT preferred, or eligibility to obtain and maintain QMHP-T registration within required timeframes.
  • Knowledge of behavioral health, intellectual/developmental disability services, crisis intervention, and Virginia’s systems of care.
  • Strong clinical assessment, communication, care coordination, and documentation skills, including proficiency with Microsoft Office and electronic records systems.
  • A valid driver’s license, current vehicle insurance, and ability to travel throughout the service area.

This position provides services in both office and community settings and requires travel throughout the service area. Candidates must be able to:

  • Lift and assist with up to 50 pounds as needed
  • Respond effectively in crisis situations
  • Travel in varying weather conditions
  • Maintain accurate documentation and administrative records
  • Work collaboratively with individuals, families, providers, and community partners
