Easterseals PORT Health is seeking a licensed-eligible clinician for the Mobile Crisis Coordinator role in Virginia. The position provides crisis intervention, care coordination, and 24/7 crisis line support in the REACH Mobile Crisis Team, with on-site or telehealth components.

Qualifications

  • Bachelor's degree in human services or related field from USDOE/CHEA accredited institution.
  • Master's degree in Counseling, Social Work, Psychology or related field preferred.
  • Current, unrestricted Virginia license as LPC/LCSW/LMFT or QMHP-T eligibility.

Responsibilities

  • Provide rapid response to individuals in behavioral health crisis.
  • Meet individuals in comfortable environments for engagement and stabilization.
  • Coordinate care, crisis intervention, and treatment planning.
  • Refer to medically necessary behavioral health services and supports.

At Easterseals PORT Health (ESPH), our mission is rooted in empowering individuals and strengthening communities. We’re seeking a compassionate and dedicated full-time Mobile Crisis Coordinator to join our Community Regional Crisis Response (CR2) team in Virginia. You must live in Lynchburg, Bedford, or Appomattox areas, and be licenses eligible.

Schedule:

Full-Time: (3) 12 hour shifts 9P-9A and (1) 4 hour shift, Sun- Wed

Position Summary: TheReach Mobile Crisis Coordinator functions primarily to provideclinical and coordination of services for REACH Mobile Crisis Team. Primary duties include on-site and telephonic/telehealth crisis interventionto include ashared rotation of a 24/7/365 crisis line. Primary duties also include crisis stabilization for individuals wh o are temporarily accessing crisis services within the individual’s home or other community setting.

Essential Job Functions:
  • Provides rapid response to individuals experiencing a behavioral healthcrisis.
  • Meets the individual in an environment where they are comfortable to facilitate service engagement, stabilization, and resolution of the crisis when possible
  • Provides appropriate care/support/supervision in order to maintain safety for the individual and others, including but not limited to crisis intervention, community stabilization, care coordination, and treatment planning
  • Refers and links to all medically necessary behavioral health services and supports
  • Coordinates with behavioral health providers providing services to the individual throughout the delivery of the service
  • Maintains linkages and relationships with community partners.
  • Ensures the completion of all required documentation for Mobile Crisis services, including but not limited to assessment, treatment planning, and Crisis Education and Prevention Plans (CEPP).
  • Participates in required meetings with REACH Team Members and other applicable parties.
  • Coordinates admissions/discharges and requests for appropriate individuals to/from REACH Crisis Therapeutic Home in collaboration with the REACH CTH Supervisor
  • Coordinates admissions/discharges and requests for appropriate individuals to/from REACH Mobile crisis supports in collaboration with REACH Team Lead
  • Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with crisis guidelines and individual’s plan.
  • Plans, structures and prompts social and leisure time activities, including activities that build skills for behavior management, stress management, communication, adaptive living and daily living.
  • Collects and enters required data and produces documentation on administrative and service delivery and overall utilization of REACH services within established timeframes and according to set protocols.
  • Complies with all ESPH policies and procedures, HIPAA guidelines, DBHDS rules, Medicaid/Medicare federal guidelines.
  • Attends all staff and program meetings asrequested.
  • Maintains regular attendance and punctuality, manages use of PTO time (if applicable), and follows all policies and procedures regarding time off.

  • Follows all ESPH policy and procedures as appropriate to position.
  • Completes required training within prescribed timeframes.
  • Performs related duties asrequested.

What we are looking for:

  • Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP)– Minimum of a bachelor’s degree in human services or related field from a USDOE or CHEA accredited institution and no less than one year of fulltime relevant experience.
  • Graduation from an accredited college/university with a Master’s Degree in Counseling, Social Work, Psychology, or closely related field preferred.
  • Current, unrestricted Virginia license as anLPC, LCSW, or LMFTor closely related preferred.
  • Must be registered (or eligible to register) as aQMHP-Twith the Virginia Board of Counseling or hold a valid Licensed Mental Health Professional-Resident or Supervisee.
  • Those not currently registered must be eligible to apply and must submit their QMHP-T application upon hire, with registration obtained within 30 days, unless they are a Licensed Mental Health Professional.
  • Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs preferred.
  • Knowledge of the MH and I/DD systems of care.
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
  • Must demonstrate capacity to effectively communicate verbally and in written form.
  • Must have a valid, unrestricted, state appropriate driver’slicense and vehicle insurance.
