Willow Terrace Nursing and Rehabilitation Center in Philadelphia, PA is seeking an RD Registered Dietitian to develop personalized diet plans for residents, collaborate with the interdisciplinary team, and promote healthy eating and quality of life.

You will assess nutritional needs, monitor progress, educate residents and families, and help manage dietary services to ensure compliance with standards and resident preferences.

Qualifications

• Must have Long Term Care experience as a Registered Dietician.
• Current Registered Dietician (RD) certification in Pennsylvania.
• Ability to work effectively with a diverse population of residents, families, and caregivers.

Responsabilités

• Conduct comprehensive nutritional assessments of residents, identifying areas for improvement and developing individualized diet plans
• Collaborate with healthcare teams to develop and implement diet plans, ensuring optimal nutrition and health outcomes for residents
• Provide education and support to residents, families, and caregivers on proper nutrition and healthy eating habits
• Monitor and track resident progress, making adjustments to diet plans as needed
• Collaborate with our food service team to develop menus and ensure compliance with dietary requirements
