Job summary

Willow Terrace is seeking a Registered Dietitian (RD) to join our Philadelphia, PA facility. You will develop and implement personalized diet plans for residents, collaborating with an interdisciplinary care team to meet health needs and preferences.

The RD will conduct nutritional assessments, educate residents and families, monitor progress, and help with menu planning and dietary compliance. A PA RD license and long-term care experience are required.

Qualifications

• Must have Long Term Care experience as an RD.
• Current PA RD license/certification required.
• Ability to work effectively with a diverse population of residents, families, and caregivers.

Responsibilities

• Conduct nutritional assessments of residents and develop individualized diet plans.
• Collaborate with healthcare teams to develop and implement diet plans, ensuring optimal nutrition and health outcomes for residents.
• Provide education and support to residents, families, and caregivers on proper nutrition and healthy eating habits.
• Monitor and track resident progress, making adjustments to diet plans as needed.
• Collaborate with the food service team to develop menus and ensure dietary compliance.
• Participate in quality improvement initiatives and stay up-to-date with best practices in nutrition and dietetics.
