Job summary

Abode Services, a leading nonprofit in the Bay Area, seeks a Service Coordinator for the Rapid Rehousing program in San Jose, CA. The role focuses on housing stability through counseling, referrals, and individualized housing plans, with duties extending to coordinating with landlords and service partners.

Qualifications

• Bachelor's degree in Psychology, Human Services, Social Work, Sociology, or related field or equivalent field experience.
• 2 years case management experience providing services to homeless or low-income individuals and/or families.
• Driving and transportation of participants when required.
• Use of personal vehicle and proof of valid California Driver's License and current auto insurance, along with a clean DMV record, is required.
• Flexible schedule to work evening and weekend hours as needed.

Responsibilities

• Provide comprehensive housing services to participants, including working with landlords.
• Coordinate services with social service partners to assist participants.
• Develop and monitor individual housing plans to promote stability and eventual permanent housing.
• Assist residents in retaining housing and accessing benefits.
• Refer for assessment and treatment services for developmental, learning, behavioral health, or addiction needs.
• Attend program meetings and coordinate with internal and external partners.
