The role requires a bachelor's degree in a related field and at least two years of case management, with a flexible schedule for evenings and weekends.

Qualifications

• Bachelor's degree in Psychology, Human Services, Social Work, Sociology, or related field or equivalent experience.
• 2 years of case management experience serving homeless or low-income individuals or families.
• Must have a valid CA driver's license and auto insurance with a clean DMV record.
• Flexible schedule including evenings and weekends as needed.

Responsibilities

• Provide comprehensive housing services and work with landlords when needed.
• Coordinate with social service partners to refer and provide services.
• Develop individualized housing plans and link to community resources.
• Assist residents in retaining housing and achieving independence.
• Document services promptly using BIRP format and maintain files.
• Attend meetings with internal and external partners to coordinate services.
