Job summary

Abode Services is seeking a Service Coordinator in Palo Alto, CA to deliver housing and supportive services to individuals experiencing homelessness. You will coordinate with landlords, refer to financial, legal and health resources, and develop individualized housing plans to achieve stability.

The role requires a Bachelor's degree in a related field and 2 years of case management experience, plus a valid CA driver license. Mon-Fri, 9am-5pm; 100% health benefits and extensive PTO are offered.

Qualifications

• Bachelor's degree in Psychology, Human Services, Social Work, Sociology, or related field or equivalent field experience.
• 2 years of case management experience with homeless or low-income individuals/families.
• Valid California driver license with clean DMV record and auto insurance.
• Flexible schedule including evenings and weekends as needed.

Responsibilities

• Provide comprehensive housing services to participants, including coordination with landlords.
• Refer and connect participants to social services and supports.
• Develop individualized housing plans to promote independent living and housing stability.
• Document services timely using BIRP format and maintain client files.
• Meet with households in the community or home as needed and participate in program meetings.
