Job summary

Delta-T Group is seeking a Rapid Rehousing Case Manager to provide services in the New Haven, CT area as an independent contractor (1099). Hours are Mon-Fri 8:30 AM to 4:30 PM, with opportunities to choose assignments.

The role focuses on identifying client needs, securing housing, assisting with vouchers, and connecting clients to essential supports, including Medicaid, mental health, and employment resources.

Qualifications

• Bachelor's Degree strongly preferred; housing case management experience valued.
• Experience in housing programs (Supportive, Transitional, Rapid-Rehousing) required.
• Ability to work with homeless populations and assist with housing applications.

Responsibilities

• Identify client needs and arrange social services for housing readiness.
• Determine housing eligibility and assist with housing voucher applications.
• Assist in locating and securing apartments for clients.
• Screen documents for homelessness and sensitive information.
• Assist with basic needs entitlements such as Medicaid, subsidies.
• Refer to mental health and addiction treatment as needed.
• Encourage social supports and community connections.
• Support employment activities: resume writing, job searching, interviewing.
• Provide ongoing case management after housing is secured.
