Job summary

Delta-T Group Lowell seeks a Housing Case Manager to support shelter residents through intake, assessment, and ongoing case management. Interview guests after five days, develop individualized plans, and coordinate housing resources to meet goals.

You will maintain HMIS records, complete documentation, and foster partnerships with community agencies and landlords to secure housing opportunities. This is an independent contractor role with flexible scheduling and weekly compensation.

Qualifications

• College degree or high school diploma/GED, depending on experience.
• 1–2 years' experience in health and human services work with homeless population.
• Haitian or Spanish language skills encouraged to apply.
• Basic proficiency in Windows, Word, and Google Email/Apps.
• Must be able to obtain and maintain crisis intervention training.
• Individuals with housing insecurity experience encouraged to reply.

Responsibilities

• Personally interview, assess, and begin case management for shelter guests after five days.
• Complete intakes, assess needs, and identify resources to address goals and housing.
• Develop individualized case plans and monitor progress via weekly meetings.
• Maintain electronic client records (HMIS) and complete required documentation promptly.
• Provide referrals and assist with applications to housing programs and services.
• Build relationships with community partners, housing agencies, landlords to secure housing.
