Job summary

Catholic Charities, Diocese of Trenton is hiring a Case Manager (Rapid Rehousing) in Trenton, NJ. You will deliver case management/counseling services to clients and families, design individualized plans, assess needs, and coordinate access to housing and related supports.

The role requires a relevant degree or comparable experience, plus state- required credentials where applicable. The position is full-time, non-exempt, 35 hours per week, with standard hours and a Monday/Wednesday/Friday

Qualifications

• Minimum qualifications include a Bachelor’s degree in a related field or equivalent mix of coursework and experience.
• Experience in human services and a valid driver’s license are required.
• Credentialed professionals (CPRP, CRC, CADC) with related experience are preferred.

Responsibilities

• Design, implement, and monitor individual program plans for clients.
• Assess client needs and eligibility for services.
• Provide counseling and assistance to clients and families.
• Maintain client records and required documentation per standards.
• Coordinate client activities and connect to community resources.
• Assist clients in accessing services (training, housing, medical care).
• Network with providers to expand available resources.
