Job summary

Delta-T Group Hartford, Inc. is seeking a Rapid Rehousing Case Manager to serve clients in the New Haven, CT area. This Independent Contractor (1099) opportunity involves coordinating social services to support clients in obtaining and maintaining housing.

The role requires a Bachelor's Degree or strong housing case management experience, and experience in supportive or transitional housing is preferred. Hours are Mon-Fri, 8:30 AM to 4:30 PM, with opportunities for flexible scheduling as part of

Qualifications

• Bachelor's Degree strongly preferred; strong housing case management experience considered.
• Case Management experience in Supportive / Transitional Housing is required.

Responsibilities

• Identify clients' needs and establish social services support for housing.
• Assess eligibility for housing vouchers.
• Help clients locate and secure apartments.
• Screen documents and protect sensitive information.
• Assist with access to Medicaid and housing/food subsidies.
• Refer clients to mental health and addiction services as needed.
• Encourage social supports and community connections.
• Provide employment support including resume writing and job search.
• Continue case management after housing is secured to support long-term success.
