Job summary

HomeFirst seeks a Tamien Case Manager to deliver comprehensive case management within the Rapid Rehousing Program, helping individuals and families avert homelessness, maintain housing, and enhance self-sufficiency through person-centered support and linked community resources.

You will maintain a 20+ participant caseload, collaborate with property management, document services per agency standards, and participate in ongoing learning to apply evidence-based practices while reporting to the

Qualifications

• 1 year paid or volunteer experience in social work or related human services required.
• Bachelor of Arts in Social Work (BASW) or related degree preferred, not required.
• Proficient in Microsoft Office suite.
• Valid CA driver’s license and insurance, and reliable transportation. Must be able to be enrolled in the Agency Fleet Program.

Responsibilities

• Maintain a caseload of 20+ participants.
• Provide comprehensive case management services including eligibility, assessments, budgeting, information and referral, crisis de-escalation, safety planning, and advocacy.
• Meet with participants as needed and collaborate with property management to support housing maintenance.
• Maintain documentation and ensure compliance with contract, agency, and licensing requirements.
• Participate in case conferencing and collaborative care coordination.
