Job summary

Father Bill’s & MainSpring is seeking a Re-Housing Case Manager in Brockton, MA to secure housing opportunities for guests in emergency shelter and outreach programs. The role prioritizes diversion from shelter and rapid housing exits, working within a Services Team to plan and assess client needs.

Requirements include 2–4 years of relevant experience or a Bachelor's in social work or related field, plus CPR certification.

Qualifications

2–4 years of relevant experience and/or social work or a Bachelor’s Degree in social work or related field.
CPR certification is required and provided by FBMS; AED and First Aid may also be required.
Ability to locate and secure housing opportunities for FBMS guests.

Responsibilities

Advocate on behalf of program guests to move them toward housing options.
Assess and redirect guests to alternatives to shelter and develop exit plans.
Help guests apply for mainstream benefits and enroll in case management programs.
Complete intake and assessment to identify strengths, needs, barriers, and goals.
Develop and maintain relationships with landlords, housing authorities, and social service organizations.
Transport guests to viewings and appointments; understand housing programs including Rapid Re-Housing.
Maintain landlord contacts and share housing leads; coordinate with outside providers.
Maintain data-based records and submit service outcome reports as needed.
Provide support, crisis intervention, mediation, and coordination; maintain confidentiality.
