Job summary

Bridges Health is seeking a Quality Manager (RN or LPN) in Edmond, OK for a long-term care facility. The role focuses on improving residents' quality of life and ensuring excellence across care processes.

Qualifications

• Must hold an active Oklahoma RN or LPN license.
• Current First Aid and CPR certification.
• Ability to pass criminal background check.
• Ability to pass POPES screening.
• Experience using EMR systems.
• Experience in LTC/LTAC/Hospice or Home Health settings.
• Basic computer skills including Microsoft Office.

Responsibilities

• Develop, implement and maintain a standardized quality management plan and program.
• Establish tracking and monitoring systems for quality improvement activities.
• Monitor high risk, high volume and unusual events in real time and retrospectively.
• Design preventive care methodologies and health care evaluations.
• Coordinate external oversight visits for contracted providers and ensure timely corrective actions.
