Job summary

Bridges Health seeks a Long-Term Care Quality Manager to improve residents' quality of life by leading a standardized quality program. The role travels between Woodward and Buffalo, OK, focusing on system-wide quality improvement in LTC and related care services.

You will develop monitoring systems, oversee external oversight visits, and lead quality initiatives across LTC, LTAC, Hospice, and Home Health settings. RN or LPN with EMR experience is preferred.

Qualifications

• Current Oklahoma RN or LPN license.
• First Aid and CPR certification.
• Ability to pass criminal background check.
• Ability to pass POPES (Post Offer/Pre-Employment Screen).
• Experience with EMR systems.
• Experience with LTC/LTAC/Hospice/Home Health.
• Basic computer skills including Microsoft Office.

Responsibilities

Long-Term Care Quality Manager | DayTravel Between Woodward & Buffalo, OK

Responsibilities and Purpose

In nursing, there is a growing need to go beyond bedside care and to look more deeply at systemic opportunities to improve outcomes and quality of life. That is the focus of a Quality Manager:

• Develop, implement and maintain a standardized quality management plan and program.
• Establish and maintain tracking and monitoring systems for health care quality improvement activities.
• Ensure high risk, high volume and unusual events are monitored concurrently and retrospectively as they occur.
• Design and develop methodologies for preventive care and health care evaluations. Conduct preventive studies to evaluate the continuity and coordination of care and to assess the quality and utilization of health care services.
• Coordinate all external programmatic oversight visits for contracted providers and ensures timely completion and follow up on corrective action plans.

Required Experience and Education

• Current and unencumbered Oklahoma Registered Nurse (RN) OR Licensed Practical Nurse (LPN) License.
• Up to date First Aid and CPR certification.
• Ability to pass criminal background check.
• Ability to pass Post Offer/Pre-Employment Screen (POPES).
• Experience with EMR.
• Experience with LTC, LTAC, Hospice, or Home Health.
• Basic computer skills, including Microsoft Office.
