Job summary

Evergreen Nephrology is seeking a Quality Improvement Specialist to bridge patients, practices, and internal teams. You will manage defined panels, deliver performance insights, and drive improvements across HEDIS and Medicare Stars measures using payer specifications and attribution logic.

The role emphasizes patient engagement, provider coaching, workflow optimization, and cross-functional teamwork within a fast-paced value-based care environment. Remote work is supported.

Qualifications

• 1–2 years of experience in quality improvement, population health, provider engagement, or care coordination.
• Patient-facing outreach experience.
• Understanding of quality workflows, gap closure processes, and documentation requirements.
• Strong communication and relationship building skills.
• EMR proficiency and familiarity with healthcare technology tools.
• Intermediate skills with MS Office Suite incl. Outlook and Teams.
• Able to work effectively in a primarily remote environment with dedicated space and secure PHI handling.

Responsibilities

• Serve as a trusted partner to nephrology and practices on HEDIS and Stars expectations and documentation standards.
• Share and interpret quality reports, care gap lists, and dashboards, translating data into improvement steps.
• Educate practices on documentation accuracy, coding completeness, and best-practice workflows.
• Facilitate recurring meetings to review quality trends and develop action plans for improvement.
• Support provider adoption of Pay-for-Performance components related to HEDIS and Stars.
• Identify risks or gaps and partner with internal teams to drive intervention.
• Manage patient panels, conduct proactive outreach to close care gaps.
