Job summary

Healthmap Solutions seeks a Registered Nurse Quality Practice Advisor to coordinate performance improvement activities across Care Navigation and the Kidney Health Management program.

You will educate providers, drive care gaps closure, and build cross-functional partnerships to improve member outcomes. The role requires RN license, BLS, 3+ years in healthcare, and willingness to travel ~50% within the assigned region. Hybrid work in Lexington, KY area is supported.

Qualifications

• Bachelor’s degree required.
• Active, unrestricted RN license required.
• BLS certification required for field roles (within 30 days).
• 3+ years in healthcare/managed care setting.
• Experience with claims or gap closure campaigns preferred.
• Driver’s license and ability to travel regionally.

Responsibilities

• Coordinate performance improvement activities to enhance health outcomes.
• Partner with Care Navigation team to improve Kidney Health Management program.
• Develop provider relationships and educate on best practices.
• Identify and implement provider Office workflow improvements.
• Support NCQA standards and timely reporting to stakeholders.
• Travel within assigned region to support practices.
