Job summary

Evergreen Nephrology is hiring a Nurse Care Manager to oversee assigned patient panels, delivering holistic, patient-centered care in a remote capacity. You will coordinate with physicians, APPs, and the IDT to address medical and social needs while guiding care transitions within a value-based framework.

Strong emphasis on compassionate care and patient empowerment. The role requires an RN with care management experience, licensed to practice, with familiarity in MS Office.

Qualifications

• Associate degree in nursing is required.
• Current RN license required; compact license preferred.
• Care management experience is required.
• Certified Case Manager certification is preferred.
• Proficiency with MS Office products including Outlook and Teams.

Responsibilities

• Manage the overall care management of patient panels using nursing experience and value-based care knowledge.
• Establish trusting relationships with patients and families to provide clinical and emotional support.
• Advocate for patients and coordinate resources within the health care system.
• Perform assessments to identify needs and translate them into patient-centric care plans.
• Coordinate interdisciplinary care to reduce fragmentation and improve CKD management.
• Monitor and adjust care management plans based on patient progress.
• Follow evidence-based guidelines and workflows to deliver high-quality, efficient care.
• Collaborate with physicians, APPs, and other disciplines to implement integrated care plans.
• Identify cost-effective measures to improve outcomes and resource use.
• Educate patients and caregivers about treatment options and care choices.
