Job summary

Hartford HealthCare seeks a QAPI Specialist to advance clinical documentation and outcomes through data‑driven quality initiatives. You will compile trends, report quality data across patient care, safety, risk management, infection control, outcomes, and satisfaction, and lead performance improvement plans.

The role requires an Associate of Science Degree with preferred bachelor’s level education and extensive experience in home health quality reporting, OASIS, and coding accuracy.

Qualifications

• Associate of Science Degree required.
• Bachelor’s degree in nursing, PT, OT, SLP, MSW, Health Administration, Management, or related discipline preferred.

Responsibilities

• Promote broad understanding, communication, and collaboration among team members to ensure appropriate use of best practices.
• Participate in the development, implementation, and evaluation of Clinical Quality improvement action plans to improve quality.
• Participate in Agency's QAPI activities, including patient incidents, employee incidents, infection tracking, and quality indicators such as OASIS and patient satisfaction.
• Continuously assess clinical staff compliance as required by internal policies and external standards of performance.
• Assist and coordinate with comprehensive quality improvement, data analysis and reporting.
• Prospectively review OASIS assessments to ensure appropriateness, completeness and compliance with federal and state regulations and organization policy.
• Ensure appropriate ICD‑10 coding and sequencing as it relates to the patient’s medical condition, including any co‑morbidities.
• Develop interventions to impact quality scores.
• Participate in delegated groups and providers on improving quality scores.
• Review corporate indicators data and other comparative data.
• Promote performance improvement plans that are consistent with cultural diversity, healthcare literacy issues and language.
• Perform clinical record audits and provide educational feedback to staff.
• Facilitate the use of evidence‑based guidelines, criteria, and other clinical tools to reduce variation in clinical practice and optimize clinical outcomes.
• Assist agency in meeting all requirements, ensuring compliance in all applicable settings, and managing reliable processes for timely and accurate submissions of required data to proper payors and regulatory bodies.
