Job summary

SCO Family of Service seeks a Quality Improvement professional to assist program staff in meeting regulatory standards and accrediting body requirements. You will lead data-driven reviews, coordinate safety investigations, and prepare action plans and reports to drive program improvements.

The role involves coordinating quarterly PIRC meetings, consumer and internal safety reporting, and contributing to reaccreditation activities. Strong data skills and effective communication are essential.

Qualifications

• Bachelor’s degree required; Master’s degree preferred in Human Services, Social Work, Health Care Administration, Public Administration or similar field.
• One year’s experience in quality improvement/program evaluation and/or administration in not-for-profit settings desirable.
• Experience with computer database and word-processing programs such as MS Office Suite to create and maintain databases, track and analyze information.

Responsibilities

• Assist program staff to meet Federal, State and local code and regulations and accrediting body standards.
• Facilitate the preparation of Annual Action Plans and quarterly reporting for submission.
• Conduct Case Record Review activities and prepare quarterly/annual reports with trend analysis.
• Coordinate Consumer Safety activities including investigations, incident tracking, and reporting with the Incident Management Unit.
• Prepare Consumer Safety reports and pass along learnings for program improvements.
• Schedule and conduct PIRC meetings at least quarterly.
• Coordinate implementation of Customer Satisfaction Surveys and related analyses.
• Prepare Internal Quality Monitoring reports and track key program indicators and improvements.
• Track Outcomes and Performance measures throughout the year and coordinate associated actions.
• Develop and utilize agency systems for data collection and trend analysis across demographics and incidents.
• Support corrective actions through policy/procedure development and staff training.
• Participate in Quality Team meetings and present results and recommendations.
• Ensure compliance with the Quality Improvement manual and update the Program Manual as regulations change.
• Assist with reaccreditation activities (COA and Praesidium).
• Complete all mandatory and assigned training requirements.
• Other Quality Improvement tasks as assigned.
