Job summary

AltaPointe Health Systems is seeking a FQHC Quality Assurance Care Coordinator to support the delivery of high-quality, patient-centered care across our FQHC clinics. The role focuses on implementing and maintaining the QI/QA program, using analytics to track performance metrics and patient outcomes, and ensuring population health data accuracy for care gap closure.

Qualifications

• Bachelor’s degree in social services/behavioral health or equivalent healthcare degree
• Minimum of two years of related experience
• Proficient with computers and healthcare software applications, including reporting tools
• Strong analytical, communication, planning and time management skills

Responsibilities

• Assist in implementing and coordinating the QI/QA program to ensure high-quality, patient-centered care
• Collaborate with clinical teams, administrative staff and payors to develop dashboards and outcome reports
• Recommend and implement quality improvement initiatives based on data analysis and metrics
• Provide outreach and coordination of preventive and chronic care services to patients
• Identify care gaps and engage patients in follow-up or care management services
• Manage payer data and clinic rosters to support care gap closure
• Generate and analyze provider/clinic performance reports
• Support data integrity through data validation and hygiene activities
• Assist in compliance and accreditation surveys (HRSA, Joint Commission, payer audits)
