Qualifications

• Bachelor's degree or higher in behavioral health, nursing, social work, or related field.
• Minimum 2 years in behavioral health, care coordination, or case management.
• Proficiency with electronic health records and care coordination systems.
• Ability to manage complex cases and engage diverse populations.
• Strong organizational and communication abilities.

Responsibilities

• Review and document clinical records to assess service utilization and treatment plan compliance.
• Coordinate physical, behavioral, and social health services across programs and providers.
• Contact and follow up after hospital discharges to ensure continuity of care.
• Monitor attendance at therapy, psychiatry, and medical appointments.
• Create, track, and close referrals in the Care Manager System.
• Identify and address barriers to care such as transportation or insurance.
