Job summary

Emanate Health in California seeks a Quality and HCC Coordinator who will oversee site HCC and STAR quality measures, train staff, and lead patient screenings and outreach campaigns. You will collaborate with the LVN Office Manager and coordinate scheduling between primary care and specialty clinics to improve care gaps.

The role requires at least a high school diploma, 3 years of office/clerical experience (supervisory), knowledge of medical terminology, and preferred Medical Assistant

Qualifications

• High school diploma or equivalent preferred.
• Knowledge of medical terminology.
• Bilingual skills desired.
• Three years general office/clerical experience preferred; supervisory experience required.
• Healthcare experience preferred.
• Experience with HCC and Quality Metrics.

Responsibilities

• Lead HCC/Quality campaigns and patient screenings.
• Train new members and direct team on Quality and HCC processes.
• Coordinate scheduling between primary care and specialty to improve care gaps.
• Provide front office duties including referrals/authorizations as needed.
• Communicate important information to patients, physicians, providers and hospitals.
