Job summary

Responsibilities

• Ensure managed care quality metrics meet targets (HEDIS, HRSA).
• Collect and enter clinical information into databases per HIPAA/HEDIS specs.
• Attend monthly QI/QA meetings and educate care teams on quality measures.
• Engage patients through outreach to close quality gaps and schedule preventive care.
