Qualifications

• Bachelor’s degree required.

Responsibilities

• Ensure managed care quality metrics meet targets.
• Collect and maintain clinical information from records and enter into databases following HIPAA and HEDIS specs.
• Attend monthly QA/QI meetings and support improvement initiatives.
• Educate providers on the importance of quality measures and performance data.
• Engage patients through outreach to close quality gaps and improve HRSA indicators.
• Coordinate PCP care following hospital/urgent care discharge and assist with QA/QI projects.
