Responsabilités

• Conduct regular audits of client case files to ensure compliance with program policies, funding requirements and regulations.
• Evaluate accuracy, completeness and consistency of intake documentation, assessments, service plans and progress notes.
• Verify eligibility criteria for homelessness prevention program are correctly applied and documented.
• Assess the quality and appropriateness of services as evidenced in case notes and plans.
• Review documentation related to financial assistance, ensuring proper authorization and tracking.
• Identify and document non-compliance, errors or areas needing improvement.
• Prepare detailed audit reports with findings, trends and corrective action recommendations.
• Communicate audit findings to management in writing clearly and constructively.
• Track and follow up on the implementation of corrective actions.
• Develop and revise program policies, procedures and forms to enhance documentation quality.
• Maintain confidentiality of client information and stay updated on laws and best practices.
• Utilize data management systems to track audit activities and generate reports.
• Contribute to QA indicators and performance metrics.
• Prescreen clients for eligibility and assist with applications for benefits (e.g., Access HRA, Housing Connect, DRIE, SCRIE).
• Close cases per HomeBase procedures and perform other duties as assigned.
