The role focuses on recovery-oriented, strength-based interventions and coordinating care with a multidisciplinary team.

Requirements include a relevant degree (Bachelor's in Human Services or equivalent with several years of experience, or Master's with at least one year) and proper licensure.

Qualifications

• Bachelor's degree in Human Services or related field with relevant experience.
• Advanced degree or licensure meets formal requirements for Qualified Professional.
• Two or more years of full-time experience with the population served; or four+ years with a non-human-services degree; or one+ year with a Master's degree.

Responsibilities

• Provide support to individuals with mental health and/or substance use issues in various settings.
• Promote recovery and independent living through strength-based interventions.
• Coordinate case management, services and referrals with clinical staff and community partners.
• Support development of daily living and coping skills and monitor progress.

Education

Bachelor's degree in Human Services

Bachelor's degree (non-Human Services) with 4+ years experience

Master's degree or higher
