Job summary

Green Oaks of Valparaiso is seeking a Full Time QMA for the 3rd shift in Valparaiso, IN. You will deliver patient-specific care per care plans, report health changes to the clinical team, and connect with residents and families in a compassionate way.

• Deliver exceptional and individualized care as determined by each resident’s care plan
• Report changes in resident health status to the clinical team
• Connect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences
• Comply with all rules and regulations regarding confidentiality and privacy of resident information
• Perform any other duties as assigned

Qualifications:

• High School diploma or GED equivalent required
• Must hold certification indicating successful completion of Qualified Medication Aide (QMA) program in the State of Indiana
• Exceptional communication and interpersonal skills
• Must demonstrate the ability to serve with love, compassion, and dignity
