Job summary

Heart of Texas Behavioral Health Network in Waco, TX seeks a QMHP-CS I/II (Case Worker) to perform case management and clinical duties in our community outpatient setting. The role includes assessments, treatment planning, timely documentation, and coordinating with agencies for admissions and discharge planning.

Qualifications

• General knowledge of mental illness and rehabilitation methods.
• Ability to conduct interviews and report results.
• Ability to work with professional groups and prepare reports.
• Must maintain health and regular attendance as per policy.
• CPR, First Aid and PMAB certification required.

Responsibilities

• Assess, diagnose and treat consumers in outpatient settings when needed.
• Document clinical services within 24 hours of interaction.
• Work with a collaborative team for diagnosis and treatment.
• Liaise with agencies, physicians and resources for admission, treatment and discharge planning.
• May supervise therapists, volunteers and students.
• participate in budget planning, unit admin and program development.
