Qualifikationen

• Bachelor's degree from an accredited college or university in an appropriate related field.
• QMHP credentialing and annual renewal required for employment in this position.
• One year case management preferred.

Aufgaben

• Complete PASRR Level I and Level II Evaluations with face-to-face interviews of individuals, families, and facility staff.
• Provide case management and psychosocial rehabilitative training per treatment plan.
• Coordinate IDT meetings and follow-up on specialized services recommended for individuals.
• Data entry of completed evaluations into the TMHP portal.
