Job summary

New Era Entities is seeking a qualified QMHP to deliver strengths-based training and interventions using an evidence-based curriculum. The role supports adults and children and works with a QMHP Supervisor to implement service plans tailored to family needs.

The position includes crisis intervention, case management, and coordination of care with various agencies. Candidates should demonstrate effective listening, empathy, and professional communication in a fast-paced environment.

Responsibilities

• Provides crisis/emergency services for child/adult/family through face-to-face contact, phone contact or on-call availability; crisis hours documented as crisis work.
• Maintains communication with client and QMHP staff about progress and clinical issues.
• Establishes client relationships, documents service needs, makes referrals, and coordinates care with other agencies.
• Uses active listening to build rapport with clients and family members; provides emotional support.
• Completes agency-approved clinical documentation within 24 hours in the EMR; late notes subject to management approval.
• Manages caseloads, delivers behavioral health services and targeted case management in alignment with regulations and protocols.
• Communicates professionally with internal and external staff, clients and others.
• Demonstrates friendliness and professional demeanor with staff and clients.
• Complies with state laws, policies, and company procedures.
• Maintains supportive relationships with client-provider and office staff; supports other departments.
• Exhibits initiative and provides expertise to coworkers and departments.
